Step 2: Fill out the Membership Form with your signature and the date
Step 3: Sign & Date the Compliance section of the Membership Form
IMPORTANT NOTE: This form is required each year for membership approval and ticket privileges

Application, payment and compliance form must be received by January 31, 2010 in order to be eligible for the opportunity to receive a Football ticket application.
NO EXCEPTIONS WILL BE MADE FOR TICKET BENEFITS AFTER THE JANUARY 31 DEADLINE.
