Step 1: Download the Varsity 'O' Roster Card & Compliance Form here (click link to download)

Step 2: Fill out the Roster Card / Compliance Form with your signature and the date

Step 3: Sign & Date the Compliance Form   

IMPORTANT NOTE: This form is required each year for membership approval and ticket privileges

Step 4: Enclose membership dues with the Roster Card & Compliance Form and send to:
